As a Certified Community Behavioral Health Clinic (CCBHC,) South Central Human Relations Center’s services are billed at a state approved rate of $304.17.
Services may be covered in full or in part by your health insurance or employee benefit plan. It is ultimately your responsibility to understand your benefit plan and coverage; any quotes of coverage are merely an estimate and NOT a guarantee of coverage or payment. We are currently accepting the following insurance
If your plan is not listed above, please call the number on the back of your insurance card to check your out-of-network benefits.
If you decide to continue with out-of-network services, please inform your provider so South-Central Human Relations Center can submit an out-of-network claim on your behalf with your insurance provider. We can also provide receipts for services for you to submit to your insurance plan or your HSA/HRA accounts administrator.
Each plan varies, so check your coverage carefully by calling the member services phone number on the back of your card and asking your insurance company the following questions:
No one is denied services due to inability to pay. SCHRC offers services on a sliding fee scale available for those who do not have access to insurance and meet income requirements.
Please review the Sliding Fee Amounts and fill out the Sliding-Fee Application. Then, please return it with proof of income and MNsure denial letter to our Patient Account Representative. Upon request we can provide a good faith estimate.
You must call to cancel your appointments at least 24 hours in advance. Failure to do so will result in a “No Show”.
If you had been approved to schedule multiple therapy appointments, this privilege will be discontinued if you “No Show” for a therapy appointment.
If you have TWO “No Shows” for therapy within 6 months, you will not be allowed to schedule a therapy session in advance. Instead, you will have to call the main number each day to see if your therapist has an opening that “Same Day”. Once you schedule and keep a “Same Day” appointment, you will again be allowed to schedule one therapy session in advance.
If you have another “No Show” again within 6 months, your case will be closed. You may call back to re-open your case 6 months after your last “No Show”.
